The following table lists all the forms and information you need for a smooth transition Back To School! Please print the forms that apply to your family and return them to your division office prior to the start of school.

Lower School: Family Handbook Students and parents should read and discuss and return the Lower School Handbook Agreement found on the last page.
  Technology Acceptable Use Policy All lower school parents and students should read, discuss and return.
  Campus Cuisine Online registration is available! For more information and how to set up a new account, click here.
  Warrior Club Pizza On Fridays, the Warrior Club sells pizza for lunch. If you would like to participate, please fill out and return this form.

Physical forms are required of all new students. Annual health records and Medication release forms are required of all students for admittance into school. Note: Deadline is August 1, 2011.
