At
St. David's School, it is our desire to provide a safe and healthy environment
for learning. Your careful attention to the following forms helps us to meet this
goal effectively. All forms should be returned to the attention of the School Nurse
by
August 1 preceeding the start of the school year.
Annual
Health Record
Who? Every student, every year. Please include
all current medications. This form is also used as an emergency contact for all sports.
Physical Form
Who? St. David’s School requires a current Physical Examination Form for all new students. The exam must be
completed within 12 months prior to the start of the school year. All students trying out or participating in the interscholastic
athletic program must have an ANNUAL physical. A physical examination after April 1 will be sufficient for all athletic
activities throughout the school year. Exam expires after 12 months.
Medication
Release Form
Who? Every student, every year. In the past, the school nurse has used "Standing Orders" from the school's consulting physician and parents consent to dispense medication. To comply with the Nurse Practice Act of the North Carolina Board of Nursing, we will now require that you and your child's physician complete this form to receive medication from the school nurse. No medications (over the counter or prescription) will be administered by either school personnel or student without the written authorization of a physician/designee and parent.
